Jan 9th 6-8 p.m Fire Dept on the corner Lincoln Rd and Railroad.

Jan 12th 10am-2pm PBLL Snack Bar - North Field

Jan 16th 6-8 p.m Fire Dept on the corner Lincoln Rd and Railroad.

Jan 19th 10am-2pm PBLL Snack Bar - North Field

Jan 23rd 6-8 p.m Fire Dept on the corner Lincoln Rd and Railroad.

Jan 26th 10am - 7pm Yuba City Mall

                                     Must have original stamped certified birth certificate, three forms of proof of residency
(must be from three different agencies)
(i.e. Drivers license, 1 utility bill, Mortgage or rental agreement.)

Fees will be $60 per child, $50 for the second sibling, $40 for the
third sibling, and free for any additional siblings. A maximum of
$150 per family.

PBLL REGISTRATION FORM or in a WORD DOC HERE you will need 4 copies of this!

MEDICAL RELEASE FORM you will need one copy.

VOLUNTEER APPLICATION you will need one for every adult who plans to coach, manager, team parent, etc.
